State disciplines health care providers            

OLYMPIA -- The Washington State Department of Health has taken disciplinary actions or withdrawn charges against the following health care providers in our state.

The department’s Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.

Information about health care providers is on the agency website. Click on “Look up a health care provider license” in the “How Do I?” section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are encouraged to call and report their complaint.

Clark County

In June 2020 the secretary of health ended probation for home care aide Alissa Anne Adams (HM60417021).

Cowlitz County

In June 2020 the secretary of health reinstated the medical assistant and certified nursing assistant credentials of Yasharah Cheyenne Middleton (CM60627665, NC60400841). Her credentials were suspended in 2019 after the Washington State Department of Social and Health Services found that Middleton neglected a vulnerable adult.

Grays Harbor County

In June 2020 the Nursing Commission charged licensed practical nurse Richard Tyler Sholes (LP60342517) with unprofessional conduct. Sholes allegedly admitted diverting narcotic medication from his workplace, and to having taken a family member’s narcotic medication.

King County

In June 2020 the Unlicensed Practice Program entered an agreement with Yan Wang that requires Wang to cease and desist from practicing massage therapy without a license, and to pay a $1,000 fine. Wang performed massage therapy without a license.

In June 2020 the Nursing Assistant Program charged certified nursing assistant Clementino Elio Lavalle (NC60675992) with unprofessional conduct. In 2020 Lavalle was convicted of fourth-degree assault – domestic violence, and of no-contact order violation – domestic violence.

In June 2020 the Massage Therapist Program charged massage therapist Sametra N. Green (MA60433865) with unprofessional conduct. Green allegedly submitted 48 insurance claims for treatment she asserted she provided to a client. Charges say the client was incarcerated on dates for 17 of the claims. In 2016 Green pleaded guilty to attempted forgery, and to second-degree attempted theft.

Mason County

In June 2020 the Medical Assistant Program charged medical assistant Lainie Katana M. Serrano (CM60505571) with unprofessional conduct. Serrano allegedly didn’t fulfill a 2019 stipulation requiring her to reimburse $500 in costs, complete continuing education in ethics, and submit a scope of practice report.

Pend Oreille County

In June 2020 the Agency-Affiliated Counselor Program charged agency-affiliated counselor Katie Renae McKenzie Green (CG60826840) with unprofessional conduct. Green allegedly invited a former client to her home for a weekend, and disclosed to her family a current client’s name and health care information.

Pierce County

In June 2020 the Pharmacy Commission conditionally granted a pharmacy assistant credential to Makira M. Koroma (VB61059036), who must complete court-ordered conditions imposed in connection with a 2020 reckless endangerment conviction.

In June 2020 the Nursing Commission entered an agreement with registered nurse Lauren E. Fowler (RN60951471) that reinstates her credential and requires her to participate in a substance abuse monitoring program. Fowler’s license was suspended in May 2020. Fowler’s Montana registered nurse license was suspended in 2019 in connection with drug diversion, forgery, and writing and/or changing physician orders without permission.

In May 2020 the secretary of health denied a registered nursing assistant credential to April Maria Adamson-Miller (NA61004932). Adamson-Miller didn’t abide by a 2015 agreement in connection with her medical assistant-phlebotomist and certified nursing assistant licenses requiring her to undergo evaluation for a substance abuse monitoring program, nor did Adamson-Miller fulfill a 2020 requirement to supply proof of having undergone a substance use evaluation.

In June 2020 the Massage Therapist Program charged massage therapist Alexandra I. Bordelon (MA60503807) with unprofessional conduct. Bordelon allegedly didn’t supply proof of having completed required continuing education.

In June 2020 the secretary of health ended probation for substance use disorder professional trainee Bruce Wayne Davis (CO60715316).

In June 2020 the secretary of health ended probation for agency-affiliated counselor Ashlee Marie Jasmin (CG60705809).

Snohomish County

In May 2020 the secretary of health denied an agency-affiliated counselor credential to Brittany Marie Robinson (CG61015254). In 2014 Robinson was convicted of criminal solicitation, use/delivery of drug paraphernalia, and two counts of third-degree theft. In 2015 Robinson was convicted of second-degree burglary, and possession of controlled substance. In 2016 Robinson was convicted of theft, and criminal solicitation. In 2018 Robinson was convicted of second-degree burglary.

In June 2020 the secretary of health conditionally granted an emergency medical technician credential to Marcus Patrick Stein (ES61028820) and ordered him to participate in a substance abuse monitoring program. In 2016 Stein pleaded guilty to reckless driving, amended from a 2015 driving under the influence charge. In 2018 Stein received deferred prosecution on 2017 driving under the influence and reckless driving charges.

Spokane County

In June 2020 the In-Home Services program filed a statement of charges against in-home services agency RJ Care Services LLC, doing business as Care to Stay Home, and Jayne Fraser and Robert Fraser, owners and operators (FS60016733). Charges say a relicensing inspection revealed eight violations of Washington Administrative Code, four of which were repeat violations.

In June 2020 the Substance Use Disorder Professional Program charged substance use disorder professional trainee Kimberlee Brooke Stoddard (CO60471065) with unprofessional conduct. Stoddard allegedly used a client’s bank card to make about $1,600 in unauthorized withdrawals. Stoddard allegedly tried and failed to deposit a $5,000 check made out to the client.

Thurston County

In June 2020 the secretary of health ended conditions on the substance use disorder professional trainee and agency-affiliated counselor credentials of Carrie Lee Moisey (CO60748903, CG60815762).

New Mexico: In May 2020 the secretary of health denied a massage therapist credential to Yi Lee Lee (MA60982384), who in 2018 was convicted on a federal charge of conspiracy to violate 8 U.S. Code 1325, improper entry by an alien.

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
