FDS Achieves Pharmacist eCare Plan Level I Capability

FDS Expands its Portfolio of Patient Care Offerings in Support of Care Coordination and Enhanced Pharmacy Services

The goal of this initiative is to empower community pharmacies to improve care coordination and health outcomes with use of electronic care plans.

Part of the Community Pharmacy Enhanced Services Network, the Pharmacist eCare Plan is a shared document detailing a patient's current medication regimen and health concerns, including drug therapy problems, therapeutic goals and medication support needs, as well as the pharmacy's interventions and the patient's health outcomes. The Pharmacist eCare Plan standard allows pharmacists to create care plans within the pharmacy's existing technology utilizing existing standards for healthcare data exchange.
