State disciplines health care providers
OLYMPIA -- The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.
The department’s Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.
Information about health care providers is on the agency website. Click on “Look up a health care provider license” in the “How Do I?” section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are encouraged to call and report their complaint.
Clark County
In January 2020 the Nursing Assistant Program dismissed a statement of charges against certified nursing assistant Yohannes Dibabu (NC60866519).
In February 2020 the Nursing Commission charged licensed practical nurse Cody Scott Kottmann (LP60853856) with unprofessional conduct. Charges say Kottmann didn’t respond to an inquiry about allegations that he slept on the job.
King County
In January 2020 the Dental Commission withdrew a statement of charges against dentist Raymond K. Chan (DE00005695).
Kitsap County
In January 2020 the Chiropractic Commission charged chiropractor Alkarim Bhanji (CH00003675) with unprofessional conduct. Bhanji allegedly provided chiropractic treatment to a family member for about four years without keeping patient records or chart notes.
Mason County
In January 2020 the secretary of health denied a registered nursing assistant credential to Stephanie Michelle Anderson (NA60989740). In 2016 Anderson was convicted of unlawful possession of controlled substance-methamphetamine with intent to deliver. Anderson didn’t supply proof of having undergone a substance use evaluation.
Pierce County
In January 2020 the secretary of health ended conditions on the substance use disorder professional trainee credential of Heather Marline Morgan (CO60891176).
Skagit County
In January 2020 the Veterinary Board ended conditions on the veterinary technician credential of Shawna Lee Biggs (AT00001350).
In January 2020 the Nursing Commission ended conditions on the registered nurse credential of Nancy Kay Rogerson (RN00160769).
Snohomish County
In January 2020 the Dental Commission withdrew a statement of charges against dentist Darin A. Vierra (DE00008171).
Spokane County
In January 2020 the Unlicensed Practice Program filed an amended notification of its intent to issue a cease-and-desist order to Maureen Ann Patrick. Patrick allegedly holds no medical, reflexology, or massage license, but represented herself as a reflexologist and colon hydrotherapist, and provided colon hydrotherapy.
In September 2019 the secretary of health denied a home care aide credential to Chelsea Lyn Montenguise (HM60774225). In 2016 and again in 2017, Montenguise was convicted of driving under the influence.
Thurston County
In January 2020 the secretary of health conditionally granted a registered nursing assistant credential to Heather Marie Johnson (NA60995343) and placed her on probation for at least 18 months. In 2019 Johnson was convicted of unlawful display of weapon.
In February 2020 the secretary of health ended probation for medical assistant-hemodialysis technician Angela Christine Coombes, also known as Angela Christine Castle (HT60362044).
Idaho: In February 2020 the Nursing Commission entered an agreement with registered nurse Cheri Susan Thomas (RN60067108) that reactivates her license and requires her to participate in a substance abuse monitoring program. Thomas’s Idaho nurse license was revoked in 2018 after she pleaded guilty to three federal felonies.
Louisiana: In February 2020 the Nursing Commission charged registered nurse Heidi Hartenstein Benoit (RN60635790) with unprofessional conduct. In 2019 Benoit surrendered her Louisiana nurse license.
Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
