Amerigroup DC and DC Connected Care Network Announce Initiative to Transform Washington D.C.’s Health Outcomes Through Value-Based Care
Amerigroup DC and the DC Connected Care Network (DC CCN) are proud to announce a joint, innovative healthcare endeavor that establishes DC CCN’s first Value-Based Agreement (VBA) with a Medicaid managed care provider in Washington, D.C. This three-year collaboration is the first of its kind in the District and is designed to transcend traditional healthcare models, prioritize patient outcomes, and set a new, higher standard for healthcare delivery across the region.
The DC CCN consists of the DC Primary Care Association (DCPCA) and seven essential Federally Qualified Health Centers (FQHCs) that are crucial to this agreement—Bread for the City, Community of Hope, Family Medical Counseling Services, La Clinica del Pueblo, Mary’s Center, Unity Health Care, and Whitman-Walker Health. In partnership with Amerigroup, these FQHCs aim to enhance care access and build upon a foundation of equitable healthcare designed to empower all residents, particularly the underserved and vulnerable populations.
A VBA links healthcare payments to patient outcomes rather than the volume of services provided, promoting a comprehensive and patient-centered approach to care.
